Meriter’s Faith Ambassador Program will present a power point presentation and lead a discussion on advance directives. Rabbi Laurie Zimmerman will present faith considerations of advance care planning specific to Judaism and will be available to answer questions.

Following the presentation trained facilitators will meet with participants in small groups to guide people through the advance directive process.  If preferred you can make an appointment to meet individually with a facilitator at Meriter Hospital to complete the advance directive document at another time.
